Feedback has been opened for a draft Strategic Plan showing how integrating health and social care services will deliver improved personal experiences and outcomes for the people who use these services.
The draft plan, which has been developed by Aberdeen City Health and Social Care Partnership through a series of integration conversations and workshops, outlines how Aberdeen City Council, NHS Grampian and partners from the third and independent sectors will work together.
The aim is to provide a range of services that fulfil the partnership’s vision of: ‘A caring partnership working together with our city communities to enable people to achieve fulfilling, healthier lives and wellbeing’.
The draft plan also contains the partnership’s proposed vision, values and strategic priorities for the next three years.
Aberdeen City Health and Social Care Partnership will have responsibility for the effective planning and delivery of those services that are in the scope of integration to meet the nine national health and wellbeing outcomes.
Jonathan Passmore, vice chairman of the partnership’s shadow integration joint board, said: “The draft plan has been produced in preparation for the integration of our health and care services on March 31, 2016.
“We are keen to hear the views of the local people about what matters to them and what they think our high quality integrated services of the future should look like. These contributions will help us shape a new way of working for the benefit of the citizens of Aberdeen.”
